Our office will assist with VA forms, please call and make an appointment. If you choose to complete on your own, make sure to make copies of everything before sending into the VA for your records.

Send Health Benefit Enrollment 1010EZ or Renewal 1010EZR Applications to:

                  Attn: Eligibility (136A)
                  601 Highway 6 West
                    Iowa City, IA 52246

You will need a Certified DD214 to send along with Health Benefit Application. Do not send your original DD214, as you will not get it back. If you have it recorded, contact the Recorder's Office for a Certified copy at no cost to the veteran. If you do not have it recorded, you should do so.
